CMS announced on November 22, 2023 that an updated version of Form 855-A is now available. The updated form was approved September 2023 and is noted on the tool as being updated 09/23. The tool revision is designed to gather more information on ownership, specifically ownership by private equity companies and/or real estate investment trusts.
