Our consultants are starting to see an uptick in the number of single-dose medication vials
being timed and dated to be discarded in 28 days after opening. Note that these are single-
dose vials and not multi-dose. That means these vials should be discarded after immediate use,
as they have no preservatives to support repeated use. The writing on these vials is hard to
read, and it is often not clear where they are labeled as single-dose medication; however, it
appears that the staff are just not looking. Ensure your staff know to review the labeling for
single-dose vials and discard them once used. Additionally, incorporate observing for single-
dose medication vials being dated/timed as part of your rounding topics for review.
