Re-Admission Penalty

In fiscal year 2020, CMS will penalize 2,583 hospitals for having too many Medicare patients readmitted within 30 days.  Re-admissions are impacting healthcare facilities in many ways. First is the impact to financial reimbursement – the 2,583 hospitals will have payments reduced by 1 to 3% based on where they fell in the rankings. Second, readmissions draws questions about the hospitals discharge planning and follow-up process.

There are new requirements for discharge planning that will take effect by the end of 2019.  In order to address the readmission rates within your organization, please review your entire continuum of care including admission, plans of care, discharge planning and follow-up processes.
