Care Enhancement

This innovative approach, developed by C&A in partnership with healthcare organizations, charges senior leadership to create an expectation and atmosphere of dynamic, expedited change.  Intensive education is provided to selected staff who become your organization’s internal Care Enhancement (CE) coaches or experts.  Care Enhancement can be offered as a stand-alone service or as a component of a Synergy Enhancement engagement.

CE coaches work with colleagues to enhance design processes to improve work flow, eliminate barriers and direct activities to the patient. This process includes an organizational assessment of current compliance and readiness for change, development of organization-specific tracer tools, education for leadership, medical staff and employees, comprehensive education and mentorship for care enhancement coaches, identification and monitoring of goals and objectives and provision of follow-up support.

C&A, in partnership with healthcare organizations, has developed an innovative approach to cultural change at the operational level to improve organizational processes and delivery systems through the development of internal experts and care coaches. This exciting approach charges senior leadership to create an expectation and atmosphere of change.

Care enhancement engagements are structured based on the size, complexity and specific needs of the organization, thus agreements for these initiatives vary in duration.

Deliverables for Care Enhancement Agreements:

  • Review and recommendations related to regulatory compliance structure
  • Assistance in selection of Care Enhancement Team
  • Education for all levels of organization including leadership, medical staff, management and tracer teams
  • Assessment of organizational challenges to achieving ongoing accreditation and regulatory compliance and adherence to key clinical and quality measures including establishment of baselines, development of strategies for improvement and facilitation of change implementation
  • Collaboratively plan interventions that foster compliance, enhance documentation and improve care coordination with a focus on:
    • Defining measurable targets with administrative staff
    • Explore documentation issues
    • TJC, CMS and State requirements
    • Core Measure requirements
    • Potential denials
    • Core Measure and Pay for Performance challenges
    • Reduction of Strategic Surveillance System Score
    • Attainment of national excellence awards: Codman, Magnet, Baldridge and others
  • Facilitate implementation of Ongoing Compliance Campaign
    • Coach and support key staff
    • Assist with change strategies
    • Monitor progress to address resistance
